Cancellations or reschedules must be made at least 48 hours in advance.
Late arrivals, missed appointments, or failure to be ready at the scheduled time will result in a Pay in Full-service charge.
All appointments are Secured with a PrePayment. Refunds if cancel is greater than 48 hours.
We understand that unexpected situations happen, but last-minute cancellations and no-shows significantly impact our ability to serve all clients effectively.

Please print and Complete these forms prior to your Initial Evaluation. I will have blank forms for you to fill out and sign upon your arrival if you need to do so, however, this may take away from your appointment time. Content to treat, Hippa, and Cancellations Policy must be read and signed prior to your Evaluation. Thank you!
